Resource Profile: Prescripción de Medicamentos (MedicationRequest) RDA

Perfil FHIR de la solicitud o prescripción de un medicamento, para su intercambio en un documento RDA en Colombia.

Orden o solicitud generada por un profesional de la salud autorizado para la dispensación y administración de un medicamento a un paciente, en el marco de un proceso de atención en salud.

La prescripción de medicamentos es el resultado de un proceso clínico mediante el cual un profesional de la salud, autorizado y habilitado, determina la necesidad de un medicamento para un paciente con base en la evaluación de signos, síntomas, diagnóstico, antecedentes y criterios definidos por la ciencia médica, con el objetivo de tratar, controlar o prevenir condiciones de salud, garantizando el uso seguro, eficaz y racional de medicamentos.

Ámbito

Este perfil define las restricciones y extensiones aplicables al recurso FHIR MedicationRequest (Solicitud o Prescripción de Medicamentos), con el objetivo de estandarizar y representar la información de medicamentos prescritos a un paciente, dentro del contexto del Resumen Digital de Atención en salud (RDA).

Ha sido diseñado a partir de los lineamientos normativos definidos por el Ministerio de Salud y Protección Social, y está armonizado con la Guía de Implementación FHIR Core Colombia.

En Colombia, la prescripción de medicamentos debe registrarse en la historia clínica del paciente, de acuerdo con los lineamientos establecidos por el Ministerio de Salud y Protección Social y con la normatividad vigente sobre la gestión de la información en salud.

Características

• Proceso clínico profesional: realizado por médicos, odontólogos u otros profesionales de salud autorizados para prescribir medicamentos, conforme a su formación y marco legal.
• Sustentado en evidencia: incluye anamnesis, diagnóstico, evaluación clínica y criterios de pertinencia terapéutica.
• Incluye detalles de la prescripción: como medicamento, dosis, vía de administración, frecuencia y duración del tratamiento.
• Se expresa mediante terminologías estandarizadas: como ATC, CUM, SNOMED CT o códigos controlados nacionales.
• Facilita la trazabilidad de medicamentos desde la prescripción hasta la dispensación.

Uso previsto

Este perfil está diseñado para ser implementado por:

• Plataforma de interoperabilidad del Ministerio de Salud y Protección Social.
• Instituciones Prestadoras de Servicios de Salud.
• Sistemas de información en salud (HIS, HCE, LIS, RIS, etc.).
• Repositorios de datos clínicos (CDR).
• Proveedores y consumidores de servicios interoperables de salud.

Se espera que sea utilizado principalmente en el escenario de intercambio del Resumen Digital de Atención (RDA).

• Se requiere el uso de terminologías controladas nacionales.
• Se aplican restricciones sobre elementos obligatorios y cardinalidad.
• Utiliza extensiones para representar información adicional relevante para el contexto colombiano.

• Este perfil no sustituye las obligaciones legales sobre el registro clínico.
• Se recomienda verificar la conformidad de su implementación.

Al momento de hacer la prescripción de medicamentos ambulatorios, es decir la fórmula de medicamentos que se entrega al paciente una vez se ha terminado la atención en salud. Esto puede ocurrir al terminar una consulta externa, y al dar de alta de urgencias o de la hospitalización. Se hará uso únicamente de los códigos DCI, en los tres respectivos RDA de hospitalización o internación, urgencias o atención inmediata y consulta externa.

La prescripción ambulatoria de medicamentos en cualquiera de estas 3 instancias se basa en los principios activos que el paciente debe tomar según criterio del profesional en salud, por lo que se debe usar la codificación DCI. En caso de que el profesional quiera sugerir una marca específica de un medicamento, puede hacerlo en el campo de observaciones. Dado que no es posible que el prescriptor sepa de antemano la marca del medicamento que le será entregado al paciente, no aplica el uso del IUM, pues su dispensación no debe estar condicionada al mismo.
