Resource Profile: Administración de medicamentos (MedicationAdministration) RDA

Perfil FHIR de la administración de un medicamento, para su intercambio en un documento RDA en Colombia.

Registro de la administración de un medicamento a un paciente como parte de un proceso de atención en salud, incluyendo medicamentos prescritos, dispensados y efectivamente administrados durante la atención.

La administración de medicamentos es el resultado de un proceso clínico mediante el cual un profesional de la salud, autorizado y habilitado, suministra un medicamento a un paciente, con base en una prescripción válida y en las buenas prácticas de administración de medicamentos, con el objetivo de tratar, controlar o prevenir condiciones de salud, bajo criterios de seguridad, oportunidad y eficacia.

Ámbito

Este perfil define las restricciones y extensiones aplicables al recurso FHIR MedicationAdministration (Administración de medicamentos), con el objetivo de estandarizar y representar la información de medicamentos efectivamente administrados a un paciente dentro del contexto del Resumen Digital de Atención en salud (RDA).

Ha sido diseñado a partir de los lineamientos normativos definidos por el Ministerio de Salud y Protección Social, y está armonizado con la Guía de Implementación FHIR Core Colombia.

En Colombia, la administración de medicamentos debe registrarse en la historia clínica del paciente, de acuerdo con los lineamientos establecidos por el Ministerio de Salud y Protección Social y con la normatividad vigente sobre la gestión de la información en salud.

Características

• Proceso clínico profesional: realizado por médicos, enfermeras u otros profesionales de salud autorizados para la administración de medicamentos conforme a su formación y marco legal.
• Sustentado en prescripción válida y evidencia clínica: con detalles de dosis, vía de administración, frecuencia y observaciones relevantes.
• Incluye medicamentos de prescripción ambulatoria y hospitalaria, con énfasis en la trazabilidad del medicamento efectivamente administrado.
• Se expresa mediante terminologías estandarizadas: como ATC, CUM, SNOMED CT y códigos controlados nacionales.
• Facilita la gestión de seguridad del paciente, evitando errores de medicación y permitiendo el seguimiento farmacológico.

Uso previsto

Este perfil está diseñado para ser implementado por:

• Plataforma de interoperabilidad del Ministerio de Salud y Protección Social.
• Instituciones Prestadoras de Servicios de Salud.
• Sistemas de información en salud (HIS, HCE, LIS, RIS, etc.).
• Repositorios de datos clínicos (CDR).
• Proveedores y consumidores de servicios interoperables de salud.

Se espera que sea utilizado principalmente en el escenario de intercambio del Resumen Digital de Atención (RDA).

• Se requiere el uso de terminologías controladas nacionales.
• Se aplican restricciones sobre elementos obligatorios y cardinalidad.
• Utiliza extensiones para representar información adicional relevante para el contexto colombiano.

• Este perfil no sustituye las obligaciones legales sobre el registro clínico.
• Se recomienda verificar la conformidad de su implementación.

Cuando se prescriben y administran medicamentos al paciente, lo cual ocurre actualmente durante la atención definida en el RDA de hospitalización o internación y en el RDA de urgencias o atención inmediata. Para registrar los medicamentos prescritos y administrados en estos eventos de atención, la resolución 1888 de 2025 indica el uso del IUM, pero dado que no todos los medicamentos tienen este código, el mecanismo acepta también la codificación CUM.

La obligatoriedad del uso del código IUM se basa en lo descrito en el anexo técnico de la resolución 866 de 2021, la resolución 1888 de 2025 y en el cumplimiento de la Resolución 3311 de 2018, que como se mencionó, define el estándar de medicamentos de uso humano en Colombia. Las IPS que administren medicamentos a sus pacientes deben implementar en sus sistemas de historia clínica electrónica el uso del IUM, el cual es asignado a cada medicamento con uso aprobado en el país por el INVIMA.

En aquellos casos en que el medicamento se administra múltiples veces durante un mismo evento de atención, se debe registrar una instancia del recurso MedicationAdministration y registrar la fecha de la última administración del medicamento.
