Perfil FHIR de la declaración de uso de un medicamento (antecedente farmacológico) por parte de un paciente, para su intercambio en un documento RDA en Colombia.

Registro de medicamentos que un paciente afirma estar tomando o haber tomado, o que un profesional de la salud identifica como consumo actual o previo del paciente durante un proceso de atención.

La declaración de uso de medicamentos es la manifestación, por parte del paciente o de un informante autorizado, del uso de uno o más medicamentos, o el reconocimiento por parte de un profesional de salud de la utilización de medicamentos, con el objetivo de complementar la historia clínica, identificar posibles interacciones, prevenir eventos adversos y garantizar la continuidad y seguridad de la atención.

Ámbito

Este perfil define las restricciones y extensiones aplicables al recurso FHIR MedicationStatement (Declaración de uso de medicamentos), con el objetivo de estandarizar y representar la información sobre medicamentos que un paciente afirma estar tomando o haber tomado, o que se identifican durante un proceso de atención en salud, dentro del contexto del Resumen Digital de Atención en salud (RDA).

Ha sido diseñado a partir de los lineamientos normativos definidos por el Ministerio de Salud y Protección Social, y está armonizado con la Guía de Implementación FHIR Core Colombia.

En Colombia, la declaración de uso de medicamentos debe registrarse en la historia clínica del paciente, de acuerdo con los lineamientos establecidos por el Ministerio de Salud y Protección Social y con la normatividad vigente sobre la gestión de la información en salud.

Características

  • Permite registrar medicamentos reportados por el paciente o su cuidador, así como identificados por profesionales de salud durante la atención.
  • Incluye detalles del medicamento, dosis, frecuencia, vía de administración, periodo de uso y observaciones relevantes.
  • Contribuye a la identificación de interacciones, duplicidades y eventos adversos, mejorando la seguridad del paciente.
  • Se expresa mediante terminologías estandarizadas: como ATC, CUM, SNOMED CT y códigos controlados nacionales.
  • Facilita la continuidad del cuidado mediante la trazabilidad de medicamentos en uso al momento de la atención.

Uso previsto

Este perfil está diseñado para ser implementado por:

  • Plataforma de interoperabilidad del Ministerio de Salud y Protección Social.
  • Instituciones Prestadoras de Servicios de Salud.
  • Sistemas de información en salud (HIS, HCE, LIS, RIS, etc.).
  • Repositorios de datos clínicos (CDR).
  • Proveedores y consumidores de servicios interoperables de salud.

Se espera que sea utilizado principalmente en el escenario de intercambio del Resumen Digital de Atención (RDA).

  • Se recomienda verificar la conformidad de su implementación.

Cuando el paciente describe al profesional de la salud los medicamentos que está tomando, es decir sus antecedentes farmacológicos. Según el anexo técnico de la resolución 1888 de 2025, estos deben registrarse bajo los códigos DCI únicamente, y se envían en el RDA de paciente.

El uso de los códigos DCI se justifica porque el registro de los antecedentes farmacológicos se basa en los principios activos que el paciente está tomando, resultando difícil que recuerde con precisión las marcas y/o presentaciones.
